Introduction

The Mpumalanga Schools Hockey Association (hereinafter referred to as "the Association") is committed to protecting your personal information and ensuring your privacy is protected in accordance with the Protection of Personal Information Act, 2013 (POPIA). This consent form outlines how your personal information, and that of the child for whom you are registering, will be collected, used, and protected during the registration process for provincial team trials.

1. Collection of Personal Information

By registering the child for the provincial team trials on behalf of the parent/guardian, you consent to the collection of the following personal information:

  • Child’s full name, identification number, date of birth, and contact details,

  • A copy of the child’s identification document and a recent photograph of him/her,

  • Parent/Guardian’s full name, contact details, and identification number (as provided to you),

  • Any medical information relevant to the child's participation in the trials,

  • Any other personal information required for the registration and selection process.

2. Purpose of Collecting Personal Information

The information collected will be used for the following purposes:

  • Facilitating the registration process for provincial team trials

  • Assessing the eligibility and suitability of the child for selection into the provincial teams

  • Communicating with you regarding trial dates, selection outcomes, and other relevant information

  • Ensuring the safety and well-being of the child during the trials

3. Protection and Security of Personal Information

The Association will take all reasonable steps to ensure that your personal information is protected from unauthorised access, use, alteration, or destruction. This includes implementing appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect.

4. Sharing of Personal Information

Your and the child's personal information will only be shared with individuals or entities involved in the selection process and management of the provincial teams. This may include:

  • Selection panel members

  • Medical staff (where necessary)

  • Relevant officials within the Association

If the child is selected for a provincial team, their personal information will also be shared with the Team Staff responsible for the team. Furthermore, the child's information will later be provided to the organisers of the tournament(s) in which the team will compete.

We will not share this personal information with any third parties without your explicit consent unless required to do so by law.

5. Retention of Personal Information

The Association will retain this personal information only for as long as is necessary to fulfil the purposes outlined in this consent form, or as required by law.

6. Your Rights and Responsibilities

You, as the registering adult, have the responsibility to:

  • Ensure that parental/guardian consent has been obtained for the collection and processing of the child’s/ward’s personal information

  • Provide accurate and complete personal information as authorised by the parent/guardian

The parent/guardian has the right to:

  • Access the personal information held by the Association about the child/ward

  • Request the correction or deletion of inaccurate, irrelevant, or excessive personal information

You have the right to:

  • Access the personal information we hold about you,

  • Request the correction or deletion of inaccurate, irrelevant, or excessive personal information

  • Withdraw your consent at any time, subject to legal or contractual restrictions and reasonable notice

7. Consent

By completing and submitting an online registration, you acknowledge that you have read and understood this POPIA consent form and agree to the collection, use, and processing of your personal information and that of the child as outlined above. DO NOT PROCEED IF YOU DO NOT CONSENT!
